The Mental Health Community Support Teams provide support to
individuals with severe and enduring mental health problems who are
subject to the Care Programme Approach (CPA) or entitled to a
service under Fair Access to Care Services (FACS) and who live in
their own homes. The aim of the service is to enable individuals to
live independently by supporting them to develop and maintain basic
living skills
There are also 12 Minimum Support Homes with an overall capacity
of 36 tenancies managed by the service in Central and South
Manchester.
The service provides:
- Support with budgeting skills such as claiming benefits,
dealing with debts and setting up payment schemes for
utilities
- Support in developing domestic routines
- Advice about home safety and security
- Liaison with other agencies such as the Department for Work and
Pensions (DWP), GP's and utility companies.
- Support in maintaining and establishing social networks
- Advice about personal care routines
- Help with activities such as meal planning and preparation and
shopping
Advice and support about daytime activity and access to day
centres, training and employment.
