The Community Rehabilitation Scheme provides residential care
and supporting housing services for people with mental health. The
scheme is a partnership between MMH&SCT & Creative Support
and aims to support individuals to achieve greater independence in
their daily lives, manage their mental health needs and achieve a
better quality of life. In 2010 this service was a finalist for the
Trust Star Awards in the Improving Health Category.
Accessing the service
We accept referrals for Manchester residents from inpatient
services, community services and a wide range of agencies
throughout the city. Once the referral has been allocated via the
City Wide Referral Meeting, an assessment of rehabilitation needs
will be arranged and an invitation to visit the service will be
offered. Service users can move into the residential care house or
supported housing depending on their level of need at the time of
assessment. If the referral is accepted then move in arrangements
will be made with the referrer and service user.

We are located close to Crumpsall Tram Station, a short walk
from North Manchester General Hospital, Cheetham Hill Shopping
precinct and all major bus routes.
Accommodation
The Community Rehabilitation Scheme provides the following types
of accommodation:
- 24 hr Staffed residential care (5 places)
- 2 Shared 2-3 bedroom Houses move on supported tenancies (7
places)
- 2 Shared 3-4 bedroom Houses long stay supported tenancies (7
places)
- 1 bedroom house move on supported tenancy (1 place)

Our approach is person centred and focuses on offering choice
and working collaboratively with individuals to achieve their goals
and aspirations. The service offers practical and psychological
support in the following areas:

The Team
The service is provided 24 hours a day by a team with expertise
in a wide range of skills and experience the team includes:
- Registered Mental Health Nurses
- Residential Care Manager
- Consultant Psychiatrist & team
- Housing Support Workers
- Occupation Therapist
- Senior Support Workers
How long will people remain with the Community Rehabilitation
Scheme?
There is no set time limit; each service user is assisted to
achieve their potential, at their own pace. When optimal
independence is achieved, move on to relevant accommodation, with
the appropriate community care package for the individuals needs,
is facilitated.
The Community Rehabilitation Scheme offers residential
rehabilitation for people with mental health problems, it is
jointly worked with a 3rd section organisation,
supporting people to achieve the greatest independence in there
lives and the management of there mental health.
