
The main function of our Community Mental Health Teams (CMHTs)
is to assess and, where appropriate, meet the social and mental
health needs of people who experience severe or enduring mental
health problems.
The Teams promote:
- Seamless care across the health and social care
partnership
- Service user and carer involvement
- A single point of access (SPA)
- Improved communication and information exchange
- Building on best practice
- The achievement of independence and autonomy
- Relationships with voluntary and third sector agencies
The service is for those people whose illness results in
substantial disability. They will be assessed as having 'critical'
or 'substantial' needs and meet the current description of being
included within the Care Plan Approach (CPA).
What do we do?
There are eight co-located CMHTs providing mental health
and social care services for adults with three in the central
locality, three in south and two in north Manchester.
There are six CMHTs providing care for people in later life with
two each in central, south and north Manchester.
CMHTs work with other clinical colleagues to offer
screening, assessment, intervention and treatments for people
who have serious mental health problems, taking into account their
health, social and personal needs.
CMHTs take the lead in co-ordinating care and
support for people on CPA and offer a range of therapeutic
interventions aimed at minimising distress and symptoms while
maximising a service user's ability to look after themselves and
promote participation in social, leisure and educational
activities. CMHTs also identify people who care for service users
and provide carers assessments.
They liaise with other parts of the health and social care
economy including the City Council, voluntary sector and other
health teams such as the Primary Care Mental Health Team. The CMHTs
also work with colleagues to meet the needs of service users
with a dual diagnosis of severe mental illness and substance
misuse. CMHTs actively participate with inpatient services to
ensure effective admission and discharge processes.
The CMHTs support access to the Independent Mental Capacity
Advocate (IMCA) within the criteria of the Mental Capacity Act and
fully co-operates in Safeguarding Adult Investigations and the
identification and protection for children and vulnerable adults at
risk, whether or not they are members of the service user's
family.
The service ensures that care and support for individuals is
maintained to a high standard throughout their time with the CMHT
and work to ensure service users are re-engaged with the community
in a timely manner.
Community Services Review of Adult Mental Health Teams
Like all other health trusts we need to
show that we are delivering high quality care to the people who use
our services in a way that gives real value for money. We also want
to ensure that recovery underpins all that we do.
The vast majority of the adults that we
care for remain in the community, supported by our dedicated
Community Mental Health Teams (CMHTs). The CMHTs work in
conjunction with people to enable them to continue managing their
mental health in their own homes, perhaps with additional support
from our outpatient, day services or other teams.
We are currently reviewing the
Community Mental Health Teams for adults (people aged under 65)
to make sure that wherever someone lives in the city they are
able to easily access the Trust services that will best meet
their needs.

Opening hours:
The CMHTs are locality based and are accessible from Monday to
Friday between 8.30am and 5.30pm. Practitioners are flexible within
these hours to accommodate other family members and friends who are
involved in the service user's care.
